Control procedures for certification certificates and certification marks

 

1 the general




In order to strengthen the management of certification certificates and certification marks and ensure their correct use, these procedures are formulated.




2 Scope of application




This procedure is applicable to the CHTC certification, in line with the certification requirements of food safety management system, HACCP system (including dairy HACCP) system, GMP system, quality management system, environmental management system, occupational health and safety management system, energy management system, GAP and organic product certification certificate/logo use management activities.




3. Management Responsibilities




3.1 The General Office shall be responsible for the printing, issuing and management of certification certificates and certification marks.




3.2 The Market Training Department and the Audit Department shall be responsible for the supervision of the use of certification certificates and marks, and summarize the results to the Director of the Audit Department.




3.3 The audit department is responsible for proposing to the general manager the handling opinions on the failure to use the certification certificate and certification mark as required.




3.4 Other departments of the company shall cooperate with the above work.




3.5 The general manager shall examine and approve the handling opinions of using the certification certificate and certification mark without following the requirements.




4 Control Procedures




4.1 Certification Certificate




4.4.1 certified




According to the "audit implementation control procedures", by the audit site audit through the technical committee assessment, the general manager approved, issued by the comprehensive office after the registration of certification certificate, certificate content is as follows;




4.1.1.1 The quality management system certification certificate shall contain at least the following information:




(1) Name, address and organization code of the authorized organization. This information shall be consistent with the information in the documentation of legal status.




(2) The address and business scope of the production operation or service covered by the quality management system. If the certified quality management system covers more than one place, the name and address information of the relevant place covered shall be stated, and the information shall be consistent with the information of the corresponding legal status certification document.




(3) the quality management system conforms to the GB/T 19001/ISO 9001 standard.




(4) Certificate number (unique identification code).




(5) The name, address and certification mark of the certification body; Other logos (such as approval logos, customer logos) can be used, but should not be misleading or confusing.




(6) Date of issuance and date of validity of the certificate.




For the recertification certificate that has not been interrupted since the initial certification, the date of the first certification certificate may be stated.




Note: When a certificate has been invalidated for a certain period of time, the certification body may retain the original date of certification on the certificate if:




- The start and end times of the current certification cycle are clearly marked;




- Label the deadline of the previous certification cycle together with the date of recertification audit.




(7) Recognition status declaration (where applicable).




(1) within the scope of the recognition of CNAS, the recognition status statement is expressed as: Accredited by China National Accreditation Service for Conformity Assessment and Accredited Registration Number by our institution.




(2) in the scope of CNAS recognition, if the customer wants to issue the certification certificate without CNAS recognition logo and (or) does not declare the recognition status, the customer must explain the reason to my organization and CNAS. An exception can be made if both our organization and CNAS consider the reason given by the customer to be reasonable and acceptable. But my organization issued by the CNAS accreditation logo and (or) does not declare the accreditation status of the certification certificate is still regarded as in the scope of CNAS accreditation.




(8) Management system standards and/or other normative documents used in the audit of certified customers, including the marking of release status (such as revision date or number);




(9) any other information required by the certification standards and/or other regulatory documents;




(10) When issuing a revised certification document, mark the replacement date to distinguish the new document from any obsolete document.




(11) Certificate query method. In addition to publishing the information on the Agency's website, the Certificate shall be marked as follows: Information of this certificate can be found on the official website of Certification and Accreditation Administration (www.cnca.gov.cn). In order to facilitate social supervision.




(12) when the international mutual recognition joint recognition mark is used. The IAF-MLA/CNAS logo should appear on the same page as the recognized certification body name and/or certification body logo and be of a similar size. The certified organization shall not be allowed to use the IAF-MLA/CNAS logo or the IAF-MLA international mutual recognition logo.




(13) Requirements for supervision and examination within the validity period of the certificate.




4.1.1.2 Food safety management system, HACCP system, GMP system certification shall cover the following basic information (but not limited to) :




(1) Certificate number;




(2) Name and address of the organization;




(3) Coverage of the certificate (including the information of the production place and workshop);




(4) Certification basis;




(5) Date of issuance and validity of the certificate;




(6) The name and address of the certification body;




(7) Recognition status declaration (where applicable).




(1) within the scope of the recognition of CNAS, the recognition status statement is expressed as: Accredited by China National Accreditation Service for Conformity Assessment and Accredited Registration Number by our institution.




(2) in the scope of CNAS recognition, if the customer wants to issue the certification certificate without CNAS recognition logo and (or) does not declare the recognition status, the customer must explain the reason to my organization and CNAS. An exception can be made if both our organization and CNAS consider the reason given by the customer to be reasonable and acceptable. But my organization issued by the CNAS accreditation logo and (or) does not declare the accreditation status of the certification certificate is still regarded as in the scope of CNAS accreditation.




(8) Requirements for supervision and examination within the validity period of the certificate.




4.1.1.3 The energy management system certification certificate shall at least cover the following basic information:




(1) Certificate number;




(2) Name, address and organization code of the authorized organization;




(3) the scope of energy management system certification coverage (the main energy production, supply and use places);




(4) Certification basis and version number;




(5) Date of issuance and validity of the certificate;




(6) Name and address of the license-issuing organ;




(7) Description of the boundaries and energy performance of the certified organization's energy management system;




(8) the supervision of the certification certificate within the validity period;




(9) The certificate information can be found on the website of Certification and Accreditation Administration (www.cnca.gov.cn);




(10) Recognition status declaration (where applicable).




(1) within the scope of the recognition of CNAS, the recognition status statement is expressed as: Accredited by China National Accreditation Service for Conformity Assessment and Accredited Registration Number by our institution.




(2) in the scope of CNAS recognition, if the customer wants to issue the certification certificate without CNAS recognition logo and (or) does not declare the recognition status, the customer must explain the reason to my organization and CNAS. (11) Requirements for supervision and examination within the validity period of the certificate.




(12) Other relevant information.




4.1.1.4 Certificate of environmental management system and occupational health and safety management system shall include the following basic information (but not limited to) :




(1) Certificate number;




(2) Name and address of the organization;




(3) Coverage of the certificate (including the information of the covered place);




(4) Certification basis;




(5) The validity period of the certificate;




(6) The name and address of the certification body;




(7) Recognition status declaration (where applicable).

(8) Requirements for supervision and examination within the validity period of the certificate.




4.1.1.5 The organic product certification certificate shall include the following basic information (but not limited to) :




(1) Certificate No




(2) Name and address of the certificate holder




(3) Name and address of the production and processing enterprise




(4) Organic product certification category




(5) Product standards




(6) certification product production information (such as the name of the base, address, area, product name, product description, production scale (planting for the corresponding product planting area, breeding for the single column maximum stock), output (unit: ton) and other information)




(7) Name, address and telephone number of the certification body




(8) The seal of the certification body and signature of the general manager




(9) Date of initial issue, date of issuance and validity of the certificate




(10) Certification of organic products shall indicate the certification status: conversion/organic




(11) Approval marks that are allowed to be used




(12) Certificate revision status




(13) A base only issues one certificate in the book, can issue more than one certificate for the organization of more than one production base (except the sub-certificate, its requirements see the requirements of 4.1.3 of this procedure).




4.1.1.6 Good Agricultural Practices Certificate shall include the following basic information (but not limited to) :




(1) Registration number




(2) Certificate No




(3) good agricultural practices certification mark




(4) Name and identity of the certification body




(5) the name and/or identity of the accreditation body (if the accreditation body is accredited)




(6) The name and address of the certificate holder




(7) Name, address and product of registered member/venue. In the case of an agricultural producer or operator organization, the name, address and products of all registered members of the agricultural producer or operator organization shall be listed in the certificate or appendix. If the agricultural producers and operators of multiple sites are granted licenses, the names, addresses and products of all sites of agricultural producers and operators should be listed in the certificates or attachments.




(8) If parallel production/parallel ownership exists, it shall be indicated.

4.1.2 Certificate No




Organic products and GAP certification certificate valid for one year, certification certificate number should be from “ Chinese Food Agricultural Product Certification Information System & RDQuo; , certification bodies shall not work out their own certification certificate numbers to issue certification certificates.




The validity period of the GMP certificate for dairy products is 2 years. The certified dairy enterprises shall be supervised and audited twice a year at least, and at least one of them is without notice. The first supervision audit shall be carried out within 6 months after the initial certification audit, and the last one can be carried out in combination with re-certification. The interval of supervision audit shall be 6 months, and the supervision audit date shall be calculated from the last day after the initial audit. The serial number of the certification certificate shall be from “ Chinese Food Agricultural Product Certification Information System & RDQuo; , certification bodies shall not work out their own certification certificate numbers to issue certification certificates.




Dairy products HACCP certificate valid for 2 years, supervision 2 times, the last time can be combined with re-certification, supervision and audit interval time for 11 months, supervision and audit date for the first audit after the last day began to calculate. The serial number of the certification certificate shall be from “ Chinese Food Agricultural Product Certification Information System & RDQuo; , certification bodies shall not work out their own certification certificate numbers to issue certification certificates.




Food safety management system and HACCP system certification certificate is valid for 3 years, the certificate validity period shall conduct three supervision, audit, supervision and audit for the first time the two stages for the first time after starting the last day of 9-12 months, two supervisory audit time interval for 12 months and can combine the third surveillance audit certification again. The number of the certification certificate shall be from “ Chinese Food Agricultural Product Certification Information System & RDQuo; , certification bodies shall not work out their own certification certificate numbers to issue certification certificates.




Quality management system, environmental management system, occupational health and safety management system and energy management system certification certificate valid for three years, the certificate validity period shall conduct supervision and audit, 3 times for the first time, supervision and audit for the first time the two stages after starting on the last day of 9-12 months, two supervisory audit time interval for 12 months, The third supervision audit can be carried out in combination with recertification. Please refer to the requirements of 4.3.3 of Document and Data Control Procedure for the principle of certificate numbering.




4.1.3 Certification requirements for sub-certificates




Management system certification organization approved by the certification can only be issued 1 certificate, if the management system certification covered by the product (or service) category and the process involved is a multi-site/multi-branch (group company), can be issued a sub-certificate.




4.1.3.1 Management system




For certification related business qualification within the scope of each place (except for temporary field, the subcontract place) or branches (group company), can issue a certificate, for the contents of this certificate must contain all the scope of the certificate of child and every place to need to have the same range, or a portion of the certificate belongs to the range of scope, the certificate should be specifically refer to the certificate.




4.1.3.2 Organic products




When there are multiple products of one base or multiple bases of one company in the same organization, sub-certificates can be issued. The requirements of sub-certificates are as follows:




(1) Multiple bases for one company: Subcertificates can be issued for each base separately, but the scope of the master certificate must include all the scope of the subcertificates.




(2) Multiple Products in One Base: The master certificate includes all the products of the sub-certificate, and the base area and production scale corresponding to the products of the sub-certificate shall be the same as stated in the master certificate.




4.1.4 Recertification issuing time and validity period of the certificate

The re-certification certificate of GMP/HACCP system shall be issued within the validity period of the previous certification. If the recertification certificate registration is not completed within the validity period of the certificate, the validity date of the certificate shall start from the expiry date of the previous certificate. The issuing date of the certificate is still the approval date of the general manager.




4.2 Issuing certification certificates and marks




4.2.1 The general office shall be responsible for issuing certification certificates and certification marks to each approved manufacturer of certified products. When approved by the production enterprise certification products put forward requirements, approved by the general manager, the comprehensive office can also be approved certification enterprises issued a small number of copies of the certificate, the copy certificate number and the original certificate number consistent.




4.2.2 When the general office issues certification certificates and certification marks to the production enterprises of certified products, it shall record the issuance and file the copies of the certificates.




4.2.3 The holder of the certification certificate shall implement the following provisions before using the certification mark:




A) The Agreement on the Use of Certification Marks shall be signed with CHTC and the use of certification marks shall be supervised by CHTC.




B) A certification mark use system shall be established, and use records shall be carefully filled in.




C) Copies of trade certificates of self-declaration of managers should be kept for at least 5 years and filed according to time and category.




4.3 Use of certification certificate and logo




4.3.1 Product certification




4.3.1.1 Organic products




Organic product certification certificate holders generally according to the demand and output certification mark, and pay the mark fee. For the use of certification marks for organic products during the transition period, please refer to the Measures for the Administration of Certification Certificates and Certification Marks for Organic Products.




Organic product certification certificates can be used for advertising, trade fairs or to promote products for publicity and display, the logo can be directly used on the product or its packaging, as an organic mark. Licensed organizations shall not print organic product certification marks without permission.




4.3.1.2 GAP




(1) GAP certification mark is divided into a certification mark and two certification mark.




(2) The certificate holder may use the certification mark in the certified products or their sales packages, product promotional materials and business activities.




(3) the certification mark can be used to scale up or down, but it is not allowed to deform or change color.




(4) When using the certification mark, the registration number shall be marked under the certification mark.




(5) The Certificate Holder shall exercise effective control over the use and display of the Certificate and the Certification Mark.




(6) Certificate holders shall not use certification certificates or certification marks to confuse certified products with non-certified products to mislead the public.




(7) When the legal entity of the certificate holder changes (such as the farm owner, the nature of the unit, etc.), the certification certificate shall not be transferred from one legal entity to another. This requires an initial inspection of the new legal entity.




4.3.2 Management system




4.3.2.1 Food safety management system, quality management system, environmental management system, occupational health and safety management system, energy management system, HACCP system, dairy products GMP system certification can be used for advertising, trade fair or promotion of products for publicity and display. The certification mark must be authorized in writing by CHTC to be used. The certification mark and the words it belongs to should not cause any confusion between the certification object and the certification granted. The mark should not be used on the product or on the packaging of the product as seen by the consumer, or in any other way construed as product conformity.




4.3.2.2 The certified organization can declare on the product package or in the attached information that the certified customer's management system has passed the certification. The packaging of a product is defined as being removable from the product and not causing decomposition, fragmentation or damage. The criteria for identifying incidental information is that it is separable or easily separable. The type label or nameplate is considered to be part of the product. The statement should in no way imply that the product, process or service is certified in this manner. The declaration should contain references to the following:




— — The identity of the authorized customer (e.g. brand or name);




— — Type of management system (e.g. quality, environment) and applicable standards;




— — The certification body that issues the certificate (for example, name or logo).




4.3.2.3 The certification organization shall correctly use the certification certificate, certification mark and audit report according to the requirements. The certification certificate shall not be transferred, sold or borrowed in any way to prevent the incorrect use, including the false statement of the certification and the misuse of the logo and audit report, which may lead to the suspension or revocation of the certification qualification. In the event of such a situation, CHTC shall investigate the fraud to prevent a similar situation from happening again.




4.3.2.4 audit department once get that units there is an error to use the certification certificate or identification information, shall be immediately respond, put the case on record, when necessary, designate or entrust relevant personnel investigation, verification, collect the necessary evidence by investigators to fill in the certificate error using processing form, the general requirements for investigating and handling completed within two weeks. The licensee shall be informed of the result of treatment and required to rectify within a time limit.




4.3.2.5 No matter what the reason, as long as the licensee stops to pay the relevant fees or is suspended, cancelled or revocation of the certification certificate, shall stop the use of any form of certification certificate and mark.




4.3.3 Identifier of recognized institution




4.3.3.1 Product certification organizations:




The accredited organization for product certification is allowed to use the CNAS accreditation mark and/or declare accreditation status on the certified product or individual package as follows:




(1) CNAS accreditation logo as follows:




















(2) Our organization shall sign an agreement with the acquired organization regarding the use of the recognition mark and/or the declaration of recognition status and supervise its use.




4.3.3.2 Certification organization of management system




Do not allow the management system certification certification organization will be CNAS recognition logo and/or recognition status statement for the product or packaging.




4.4 management




4.4.1 Certificate holders must accept CHTC's confirmation of the validity of their certificates at least once a year according to the Post-Certificate Supervision and Management Control Procedure and allow them to continue to use the certificates. The use and management of organic product certification marks shall be based on the Organic Certification Certificate and Certification Mark Use Management Measures; During the validity period of the certification certificate, the holder of the certification certificate shall apply for a new certificate under any of the following circumstances:




A) The name of the production or processing unit of organic products or the nature of the legal person is changed;




B) a reduction in the type and quantity of the product;




C) The conversion of organic products has expired;




D) Other circumstances requiring changes.




4.4.2 Within the validity period of the certificate, it is allowed to use the certification certificate and mark. The replacement, expansion, reduction, cancellation, suspension and cancellation of the certificate and mark shall be in accordance with the Control Procedure of Approval, Maintaining, Renewal, Enlargement, Reduction, Suspension, Cancel and Restoration of Certification Registration.




4.5 Change of certification certificate




4.5.1 The certified enterprise that intends to change the certificate shall submit "Application for Certification Change" and relevant materials to the Market Training Department or the Audit Department, and the contents of the certificate change shall be clearly specified in the materials.




4.5.2 After receiving the application materials of the certificate modification requested by the certified organization, the audit department shall review the application for the certificate modification and judge whether it meets the following conditions for the certificate modification:




A) The content and data of the Certification Change Application are complete;




B) The contents stated in the copy of the original certificate are authentic;




C) The obtained certificate is not in the suspension period;




D) The obtained certificate has not been revoked and the certificate is still valid;




E) Professional projects fall within the business scope recognized by CHTC.




4.5.3 Whether the application for certificate change is accepted or not shall be examined and approved by the Audit Department.




4.5.4 On the basis of meeting the above conditions, the inspection method shall be determined according to the changed items and specific contents:




4.5.4.1 If the products after the certificate change can be determined according to the written information to meet the corresponding certification standards and legal and regulatory requirements, there is no need to arrange on-site inspection, such as:




A) If the application for a change of certificate of the certified organization only involves a change of the name of the organization;




B) The organization does not implement the relocation, but only applies for the change of address name (such as place name, road name, etc.).




4.5.4.2 if necessary, the on-site inspection should apply for the certificate of affirmatory select the following changes and check way: combining authentication certificate change checks again, or arrange special inspection change certificate, according to the change of inspection methods, product coverage and at the same time and with the last check check time interval, and so on and so forth to determine.




4.5.5 Implementation of Certificate Change




4.5.5.1 for the written examination can change the content of the certificate of project, contract review audit department needs to arrange personnel to review the effectiveness of the change material, authenticity and integrity, when necessary, please added by the relevant proof materials, directly submit a technical committee, organized by technical committee evaluation to assess the certification decision after personnel for approval.




4.5.5.2 For the items requiring on-site inspection to change the content of the certificate, it shall be carried out in accordance with the Audit Implementation Control Procedures. For the certified organizations applying for the expanded scope of certification, the inspection plan shall include all the management activities and areas involved in the expanded scope of certification.




4.5.3 When the scope of certification is narrowed, the certified client organization is required to modify all advertising materials.




4.6 List of Licensed Customers




The company establishes an online directory of certified customers and publishes the certification information of all certified customers on the Internet to ensure that the directory is provided upon request. The directory includes the following information: the name of the authorized customer, relevant regulatory documents such as certification standards, certification scope and geographical location (such as country and city) or the geographical location of the headquarters and all locations within the scope of multi-site certification.
